What if I am already scheduled or started treatment with a different provider?

What type of information may MedRisk/SPNet contact me about during the scheduling process?
We have been provided with your claim information, however, there may be times we contact you to confirm the following to avoid delays in scheduling:
- Your physical address
- Your workers’ comp insurance carrier
- Injured body part
- Therapy service
- Jurisdiction state (state you were injured in)
- MD information: name and phone number
What should I bring to my appointment?
Please bring a Photo ID as well as your doctor’s order for physical medicine (if you have it). If you do not have it, we will ensure a copy is sent to your provider.
What if I need to change my appointment day/time?
Please reach out to your provider directly to request a new date and time or call us at 1-800-992-9675. If the provider does not have an appointment date and time that works with your schedule, call us for further assistance.
Why did MedRisk/SPNet schedule my initial evaluation only when I am authorized for multiple visits?
We help find the right provider for you and schedules the first appointment. For your convenience, subsequent visits may be scheduled with the provider directly when you attend the initial evaluation.
Who do I reach out to with questions regarding my claim? (For Example: mileage reimbursement, compensation payments).
Worker’s compensation payments and reimbursements are handled directly through the worker’s compensation insurance carrier. Please reach out to your adjuster or claims manager for details.
